Abstract
This research examines the relation between structure and quality of Basic Care of a hospital. At this moment, Basic Care varies in quality at the hospital. It is possible that the structure influences quality of Basic Care, which is the focus of this research. This situation impedes the realisation of the vision and mission of the hospital to deliver high-quality Basic Care.
This thesis studies how structure contributes to solving quality problems and increasing quality of Basic Care. The following central research question is answered: What is the relation between: 1) gaps between the normative and actual quality of Basic Care, and 2) gaps between the normative and actual structure, and what are the characteristics of a structure that increases the quality of Basic Care of the hospital?
Theory presents that Quality of Basic Care has no generally accepted definition. Although, for the purpose of this research the following variables are used: self-empowerment, safety, effectiveness, efficiency, timeliness and guidance. The Sociotechnical Design Theory particularly discusses the relation between structure and quality aspects. The following parameters are applied to this research: functional concentration, specialisation of operational transformations, and separation between operational and regulative transformations. Document analysis, observations and interviews are carried out with two pilot units in order to examine the relation between structure and quality of Basic Care.
