Abstract
The purpose of this thesis was to investigate how designers cultivate the installed base while (re)designing the guideline for health information exchange (HIE) standards in acute care in the Netherlands. The guideline serves as the foundation for the information standard, which requires standardization in acute care processes. The installed base refers to the existing standards and practices connected to large information systems.
Data was collected through interviews with 10 designers of the guideline, using an abductive, qualitative approach. The study identified phases in the design process alongside three trade-offs associated with the installed base.
The research identified several cultivation strategies employed by designers. These included proactive support and guidance, piloting, incremental changes and celebrating small gains, process adaptation, and lobbying. These strategies were applied in specific phases of the design process and addressed different trade-offs.
Overall, the trade-offs and cultivation strategies aligned with existing literature, with some additional strategies identified. Further research is recommended to explore these cultivation strategies longitudinally and assess potential changes over time.
